All Coastal Bay Managers and Coaches must fill out and submit the following THREE forms, along with a copy of your Driver's License.

Two-page ASA background check form (same form for 2011, please re-submit to us, using THIS form):

And, please make 2 copies of your Driver's License to send in as well.
